Dr. Berry and his staff will be out of the office Friday, July 3rd for Independence Day. Any prescription refills or questions submitted during this time will be addressed on Monday, July 6th when the staff will be back in the office. If you have an urgent health question that cannot wait until Monday you may reach the physician on call at 801-491-1459. We apologize for any inconvenience this may cause and appreciate your understanding.

Dear Central Utah Clinic Patient, (Cascade Family Medicine)
On Friday, the 5th of June we sent you a letter saying that the online messaging capabilities for Dr. Berry would be changing.
This is a reminder that starting Monday June 22, 2009, Central Utah Clinic, Dept. of Family Medicine will be using iHealth for all your online messaging needs.
Switching our providers to iHealth will allow you to communicate with your doctor via a secure Internet format.

Once you have gone here, you will be directed to the iHealth portal to sign up with Dr. Berry.
With the new Online Services you will have access to ask general questions, request prescription refills, ask for appointments, and complete a Personal Health Record for you and your family.
